Parts A & B are needed yearly for ALL Participants - Applies to all trips such as local tours and weekend camping less than 72 hours.

Part C - pre-participation physical is needed for any event 72 hours or more.

This Opt-Out form shall be in effect for one year from the date signed. We understand that some people may choose not to have their photos used and we want to respect their wishes. Therefore, we have provided this “Opt-Out Form” for those individuals or families who prefer NOT to have photographs of themselves used by the Troop. Complete and submit this form ONLY IF YOU DO NOT WANT your photograph used by the Troop.
